A Family Nurse Practitioner (FNP) is a Registered Nurse who has completed additional advanced nursing education such as a master's degree or doctoral degree, as well as additional training in the diagnosis, treatment and management of a wide range of common to complex medical conditions.

How do I schedule an appointment for an initial exam or a sick visit?
What information do I need for my first visit?
Please be ready 10 minutes early for your appointment. To assist us, please have your photo ID, as well as a list of any medications, vitamins and/or herbal supplements you are currently taking ready. Please also be prepared to pay prior or at the time services are rendered.
What if I don't have health insurance?
We are currently not accepting insurance. We request that payment be made at the time services are rendered. You may contact our financial coordinator with questions concerning your account or to develop a payment plan that meets your needs.
How can I get a prescription refill?
Contact us and request an appointment for a prescription refill. If the medication is able to be refilled, we will submit a request to the pharmacy of your choice.
Please keep in mind that some prescriptions may require a follow-up exam to determine the need of a refill.
No Show Policy: We understand that things do arise but after the third (4) missed appointments the patient will be dismissed from the practice.
PRESCRIPTIONS
We ask that you bring all prescription medication(s) with you to your appointment, even if it was prescribed by another physician. Please request all refills of continuous medicines during your appointment.
What Is Vitamin IV Therapy?
